The practice is now a specialist aged care service due to a welcome significant increase in nutrition standards under the New Aged Care Quality Standards. We provide support for Canberrans living at home or in aged care.
AGED CARE SERVICES - Our team of aged care dietitians continue to support aged care facilities across Canberra. Our services include individual resident assessments, case conferences. We can then recommend individual meal plans, to meet the specific needs of residents. We can also prescribe High Protein and high energy supplements, and manage PEG ( tube) feeding.
FOOD SERVICE EXPERTS - We're experts in working with facilties in undertaking the mandatory Meal time and Menu reviews.
EDUCATION - Staff education is also mandatory under the new standards. This can include educating staff in key topics including Malnutrition, Assisting residents at meal time.
AUDITING for COMPLIANCE - As onsite dietitians, we always have our eyes open to assist facilities and staff to nourish residents with their preferred choices including compliance with texture modified ( IDDSI) meals and drinks.
LIVING AT HOME - We support you as well.
This can include a private nutrition assessment referred by your GP or geriatrician.
Nutrition / Meal support is available under the Commonwealth Home Support Program, the Support at Home program, and the Transition Care Program. A dietitian referral can be made under all of these packages. Speak to you home care provider about a referral and your avaiable funding.
AGED CARE REFERRALS
Caroline and her small team of Accredited Practicing Dietitians continue to provide a range of dietetic services for the majority of aged care providers across the ACT and surrounding region.We provide individual resident assessments; nutrition care planning; liaison with GPs, Geriatricians and transferring hospitals; as well as support for nursing and food service staff to nourish residents in an enjoyable and clinically safe manner.We also work with food service staff to improve compliance with quality standards and IDDSI texture modification, with a focus on food first, to reduce supplement use where we can. We are also happy to provide education for nursing and food service staff on a variety of topics including malnutrition screening, IDDSI, and enteral feeding.
All of our staff have the necessary Dietitians Australia membership, Professional Indemnity Insurance and Police checks.
